Emergency Recall Notice

IMPORTANT NOTICE

 Ocean Technology Systems Statement

"Due to the severity of the recall for the M & J Engineering swivel and the potential that the swivel could cause a serious diving accident, Ocean Technology Systems has decided not to distribute and/or sell the swivel any longer. Our recommendation to anyone who has a M & J Engineering swivel is to NOT use it unless you are 100% sure it is in good working order. We recommend you contact M & J Engineering for questions, concerns, recall service and/or general service for this item."

In a recent meeting with M & J Engineering Staff on August 15, 2005 (hereinafter known as M & J), it was determined that the swivels have a manufacturing tolerance problem that needs to be corrected. The manufacturer, M & J suggests that you connect the swivel to a Interspiro 2nd stage regulator (which is what the said swivel was designed to operate with) and hand tighten the swivel connector to the 2nd stage regulator. After tightening and turning on your air supply, you should feel a .010 play as you try and move the hose in & out from the swivel. If you experience no play at all, your swivel needs to be replaced. DO NOT USE THE SWIVEL IF YOU ARE NOT 100% SURE IT IS SAFE!

Recall Notice - M & J Swivel - 8/11/05

M & J has issued a Recall notice for all Swivels manufactured during the time period of January 2004 through February 2005. This notice is for the M & J after market swivel designed to be used with the Interspiro Divator MKII full face mask and a standard diving type supply hose.

DO NOT DIVE ANY M & J SW-3000 (SW-1 stock number SR-109-AGA) swivel until further directed by M & J Engineering and especially if you have noticed the swivel unscrewing during normal use in or out of the water!

 

The M & J swivel pictured above is connected to an Interspiro 2nd stage regulator and has a standard scuba type hose connected.

OTS Unofficial Test: We have received reports from the field that swivels have come loose causing divers to take emergency actions. We conducted a test on one of the SW-3000 (SW-1) swivels. We connected a HSA-2 Interspiro type supply hose to the swivel and connected the swivel to a Divator MKII 2nd stage regulator. We initially found that it worked fine moving in all directions. The first appearance was that it was OK. After applying pressure (turned on the air supply to the system) the connections became stiff. After moving the supply hose around, the swivel began to start a ratchet type action. It actually started unscrewing with each movement. We also got the connector to move without pressure to the system.

 

If you have experienced a problem or concern with a M & J after market connector, within or outside of the recall date, please contact M & J immediately and DO NOT DIVE OR USE THE SWIVEL.
